Subject: Organizing Health Check up campaign at Brigade Millennium

We request you to grant permission to conduct a health awareness campaign on the 13th of Mar 2010.
The campaign is intended to get medical details for a survey given out free Medicare cards for the benefit of the residents. Kindly oblige and do the needful.
The Campaign consists of:
1. Free Health check up.
2. Distribution of Medicare cards(upto 30% discount on any treatments in BGS)
